POLICY FOR NEW CLIENTS

If you have any of the following documents, please send them to me by email before attending the first session:

The referral letter from medical doctor, lawyer, psychologist, teacher, nurse, social worker, etc.
The most recent medical discharge summary, inpatient and/or outpatient
Neuroimaging reports
Previous psychological reports
Any other documents that you think would help me understand more your conditions

These documents will help me have a better understanding of your situations before our first meeting. they also allow me to plan for a more efficient meeting with you.

If you cannot send the above documents before the first appointment, please bring them along during your first visit.
